Chronic Condition Management (CCM) Plan
If you have a chronic condition, you may be eligible for Medicare rebates towards physiotherapy or exercise physiology sessions at X10 Physio through a GP Chronic Condition Management Plan, previously known as an EPC or CDM plan.
What is a GP Chronic Condition Management (CCM) Plan
If you have a chronic condition managed by your GP, you may be eligible for Medicare rebates towards physiotherapy or exercise physiology sessions. Medicare will contribute towards the cost of seeing an allied health professional if you have a chronic medical condition, that has been present, or is likely to last, for 6 months or more. From here your GP will prepare Chronic Condition Management Plan for you to bring into clinic.
How to get a GP Chronic Condition Management Plan Referral

Book an appointment with your GP and discuss your condition

Your GP determines eligibility and creates the plan

They issue a referral letter to X10 Physio — bring this to your first appointment

Bring your Medicare card and any relevant reports or test results your GP has provided

How many physiotherapy sessions can Medicare cover?
The GPCCMP entitles you to access up to 5 Medicare rebates for 1-on-1 allied health services in a calendar year. You can direct all 5 sessions to one discipline (e.g. all physio, or all exercise physiology), or split them across different allied health types.
How much is the Medicare rebate for physiotherapy?
From July 1, 2025, the Medicare rebate is $61.80 per session. Most clinics charge above this, so a gap fee typically applies — we'll be upfront about this at X10.
How long is a GP Chronic Condition Management Plan Referral valid?
Referrals are valid for 18 months. Some patients will require a new referral each year as their plan is reviewed and updated — your GP will confirm this at your appointment.
